Health facilities are increasing their pressure to improve patient outcomes globally. This occurs as patients increase their awareness of their overall health demand, making them more selective to healthcare facilities. Many patients are looking for treatment at facilities with a reputation for superior care. Hospital management must understand that treatment outcomes are the essential services that can make the organizations succeed and have a competitive image worldwide. Therefore, some measures need to be implemented in health care to ensure patient care practice is improved.

Well-informed diagnosis is vital in ensuring patients’ treatment process is successful. A successful treatment process starts with a correct and timely diagnosis. When patients get into the healthcare facility, the care providers should ensure they assess the patient’s health problem from the past treatment history and analyze the possible cause of the current situation. This will help the care provider have advanced knowledge of the patient’s health condition and make an effective treatment process.

As a result, the patient care practice will be improved by promoting a well-informed diagnosis of patient conditions (Mackey & Bassendowski, 2017). Diagnosis mostly tends to be challenging as human error can occur or mistakes from the systems. Besides, lack of enough staff can increase diagnosis errors. However, if the healthcare facility enhances its resources, it will be easy to improve the patient care practice through the well-monitored diagnosis process.

Optimal treatment planning can enhance patient care practice improvement. After diagnosis, care providers should ensure they select the best treatment process that will increase patient outcomes. Through evidence-based practices, caregivers can implement the best treatment method that will positively impact patient care practices. Healthcare facilities then should ensure they recruit care providers who are well experienced and can follow all procedures directed by the diagnosis results.

This will ensure errors in treatment and medication are dodged and increase the patient care practice within the facility (Kuijpers et al, 2016). The use of modern treatment observation has added advantages in patient care practices. Hospitals should ensure they implement the modernization where technology is applied to provide and secure patient treatment.

Healthcare facilities should ensure the clinicians are cared for and provided with all requirements they need during their working hours. Clinicians are care providers and all other staff with the facility who significantly impact patient care practice. Facilities with good care can improve their patient care practices as care providers are satisfied committing more in their role-play (Zarowitz, Resnick & Ouslander, 2018).

Mostly, the healthcare facility should ensure there is a friendly environment and clinicians are motivated through training and seminars that can enhance their skills. For instance, better staffing with movement towards patient care practices can encourage care providers to give quality care practices. This will help the care providers improve their services and ensure patients are attended to and given the best experience to enhance the care practice process.

Patient-centered care is paramount in the improvement of the care practice. Care providers should utilize their working hours in providing the right services to the patient. The treatment methods should be preserved with dignity and respect and allow the patient to participate in the decision-making process. According to Hochman (2017), nurses and care providers need to involve the patient in their treatment decision after the diagnosis is made.

This will help improve the patient care practices and reduce the unnecessary argument between the patient family and the clinic. Besides, to improve the patient care practice through a patient-centered approach, the care providers have to provide emotional support and alleviate fear and anxiety. This increases the unity and bond between the healthcare facility and patient, promoting the care practice services within the facility.

Health facilities aiming at improving care practices have to ensure follow-up care of patients even after being discharged. Mostly, health care facilities did not make treatment follow-up of the patient after releasing them or transferring to other hospitals. This is a weakness in improving patient care practices. Patients need to see some kept communication from their care providers and the facility management (Rivera-Lebron et al, 2019).

This will motivate the patient and even promote quick recovery. The hospital has to foster communication and coordination between care providers and patients even after discharge and ensure the patient recovery process is monitored to avoid readmission. Patients’ needs to be educated on the importance of follow up communication with their specialists as some can neglect which can increase health complication. Follow-up after discharge has a high ability to improve patient care practices.

Conclusively, patient care practices are vital to all nurses and care providers. Improving patient care practices, as seen above, needs more consideration on the healthcare facilities’ insight into the patient outcomes. Patients need to have a well-informed diagnosis followed by an optimal treatment plan that keeps the results and diagnosis process in front. Besides, caring for the care providers motivates them to improve their quality of treatment and care practices. Healthcare facilities also need to ensure the service offered is patient-centered and a follow-up of patients after being discharged from the hospital. The patient care practice will be improved through all this, and the facility continues to anticipate more success.
