Implementing the American Heart Association Diet and Lifestyle Recommendations aims to Promote Healthy Behaviors and physical activity, reducing sedentarism and curbing obesity trends. A 1% reduction in the predicted obesity trend, according to the Centers for Disease Control and Prevention (CDC, 2021), would result in 2.9 million fewer obese persons by 2030, saving $4.7 billion annually…

In overweight adult patients in a primary care clinic, what is the impact of implementing the American Heart Association Diet and Lifestyle recommendations, compared to standard care, on body weight in 8-10 weeks?

·       Discuss best practices for implementing evidence-based guidelines with references to support best practices.

·       Discuss strategies for overcoming barriers in implementing evidence-based practice with references sufficient to support strategies.

·       Discuss your formative evaluations.  How are you addressing challenges and providing feedback?

Staff will call participants weekly to reinforce the use of the Fitbit app and answer questions or concerns about the intervention. Weekly chart audits with participants’ calories and tracking of physical activity compliance will be used as a formative evaluation…

·       Describe your sustainability plan (answer each point independently using references to support your answer):

To sustain the use of the American Heart Association Diet and Lifestyle Recommendations is needed encouragement and proper patient education on the advantages of regular physical activity. Also, it will be helpful to reinforce nutritional knowledge, positive patient and provider attitudes, access to healthy food, and eating self-control, as these are significant factors in fighting overweight and obesity. Knowledge, attitudes, and dietary self-regulation are critical predictors of overweight and obesity to maintain long-term weight management (Balani et al., 2019)…

· Discuss how the stakeholders will maintain intervention fidelity moving forward.

· Are champions appointed to assure sustainability?

· Are there required changes to the technology to sustain your project?  If so, how will your champions advocate for these changes?
